1. Overview & Purpose
Form 65/J (Submission Eligibility Verification Request) is a prerequisite authorization instrument used to confirm that an applicant is administratively eligible to engage with Infinidim Enterprises' formal service commissioning processes. It does not commission a service. It establishes that you are permitted to commission a service.
Without a processed Form 65/J on file, no other form submitted by the applicant may be actioned, stored, acknowledged, or reviewed. This includes all forms in the Infinidim Enterprises Regulatory Filing Registry.
Important Notice — Read Before Proceeding
These instructions total four pages. Submission of Form 65/J by an applicant who has not read all four pages constitutes a self-executing violation of Administrative Sub-clause 7(b) and will result in automatic disqualification without the right of appeal. By completing the form, you certify that you have read all pages.
2. Who Must File
Form 65/J is mandatory for any party who:
- →Intends to commission a bypass, demolition, archival, or appeal service from Infinidim Enterprises
- →Seeks to register awareness of an active public consultation project
- →Requires a Notary Locator Reference (Form NLR-1 also requires Form 65/J as a prerequisite)
- →Wishes to access any Infinidim Enterprises administrative document or filing
- →Has been directed here by any other Infinidim Enterprises form
3. Before You Begin
Ensure the following are in order before completing this form. Incomplete submissions will be rejected without notification:
- ①Valid galactic identification (Form ID-7 or inter-sector equivalent, not expired within the last 3 stellar cycles)
- ②Your home sector reference code in standard XX-0000-X format
- ③A notarized copy of Form 23/Beta (Authorization of Submission Intent). See Page 2 for documentation requirements. See Page 3 for notarization procedure.
